Dear Redlands TAD Welfare Office,

 

My name is Carrie Elizabeth Johnson (SSN#) a former receiver of the California TANF/CalWORKs program. While receiving public welfare such as food stamps and cash aid, I need to request full records of the dates and amounts received for the entire time that I was receiving TANF. If you have records of the mandatory courses that I have attended, like resume classes or can furnish a letter on a TAD Welfare Letterhead giving the dates and classes that were required of TANF receivers to attend, while receiving public benefits then that would be great.

 

Secondly, I received child care assistance for the appropriate needs such as employment or pursuing a higher level of education and not professional networking which is not an immediate need and could be worked around such as virtual professional networking anyway. However, I would like to obtain those records for formal complaints that I will be filing on both a state and federal level, regarding the home daycares that I used for my daughter while I was either working, unemployed job search hours or attending college courses.

 

I can sign any release of consent form that you provide to me by email, as I have not been a resident for the State of California since September 2016 and have not returned to the State of California since. Attached is proof of my old California State ID and current ID to verify my identity. Thank you in advance and I appreciate your speedy response to processing my request by email. Please see all TANF/CalWORKs records and Letter of Attendance or Letter of Proof of dates received for public benefits, amounts and end of benefits to my email address listed above.
