Individual Tax Records for either State, Federal or Both Tax Records on Carrie
Elizabeth Johnson
Dear Missouri Department
of Revenue,
This is a formal
written request for tax records that I filed while living in the State of
Missouri from a time frame of 2004-2007 and I have not stepped foot into the
State of Missouri since. This is an individual request of my own previous tax
returns and that I have not give anyone any power of attorney to request tax
records on my personal income tax returns for the years that I was living in
the State of Missouri from 2004-2007. I have never owned a company in the State
of Missouri and have no intentions of ever starting a company in the State of
Missouri either. In response to my email sent to income@dor.mo.gov , I received the
following forms 1937 and form 2827 to be completed and emailed back in order to
have my request processed.
Attached, you will
find the following forms 1937 and form 2827 both completed to have my individual
request processed. Once my individual tax returns request is processed then I
would like those records to be emailed to me at [personal email used] . Some helpful information
that may be useful in locating the individual tax returns would be some of the
names on the W-2’s of some past employers that I filed tax returns while in the
State of Missouri would be the following: Black Angus Steakhouse (CA-Company
for beginning of 2004 tax year filed in 2005 in Missouri), Missouri National
Guard Active Duty Service Workers, Forever 21 Retail Store and Wet Seal Retail
Store that I worked for at the Saint Louis, MO. Galleria Mall, some security
company, Today’s Staffing and some other staffing company that I vaguely
remember.
The following
addresses that either were used or may have been used when filing my individual
tax returns while residing in the State of Missouri would be the following
address:[omitted from this post] would be past addresses used on my individual tax returns.
Please see the attachments and I can be reached by email that is in the header
